Scientific data (oral ALA intake)
In clinical trials of oral alpha-lipoic acid (ALA), the most notable results were shown in diabetic polyneuropathy and metabolic parameters: in the double-blind RCT SYDNEY 2 (5 weeks), ALA 600–1800 mg/day reduced the severity of sensory symptoms (Total Symptom Score) by an average of 48–52% versus 32% in placebo (for 600 mg: −4.9 points / −51% ; “response” ≥50% symptom reduction: 62% versus 26% ). In a placebo-controlled pilot RCT in patients with T2DM, 4-week oral ALA (600–1800 mg/day) was associated with a change in insulin sensitivity (MCR in the clamp test) of +27% vs placebo . According to a systematic review and dose-response meta-analysis of 16 RCTs (n=1035), each +500 mg/day of oral ALA was associated with an average reduction in HbA1c of −0.17% and fasting glucose of −6.08 mg/dL (the authors note that the mean effects were statistically significant but generally small). Scientific data in sports (oral ALA)
Support for recovery/maintenance of strength during a “hard week.” In a double-blind, randomized, crossover study of 17 well-trained men during a 6-day intensive microcycle (4 strength + 2 runs), alpha-lipoic acid was administered: 150 mg 2 hours before training + 150 mg immediately after (total 300 mg/day ). After 6 days, the placebo group had significantly lower 1RM back squat strength (shown 125.6 ± 27.2 kg ; p=0.032), while the ALA group maintained strength at baseline (shown 130.0 ± 27.2 kg ; p=0.320). Oxidative stress after muscle-damaging exercise. In a study in trained men, 8 days of oral ALA supplementation at 600 mg/day was associated with a reduction in markers of oxidative damage: the authors reported that in the trained group, ALA reduced TBARS and protein carbonyls by more than 30% compared to controls, and also affected parameters of the glutathione system after exercise (reported: GSH +40% , GR −24% , GPx +29% ). Endurance/anaerobic work: effect on lactate, no “ergogenic” gain. In a 4-week double-blind RCT, the combination (including ALA 300 mg/day ) was used in 41 trained cyclists. Results: Performance measures (Wingate/aerobic) did not improve compared to placebo , but after one of the tests the authors recorded a decrease in lactate of 1.5 mmol/L at POST (p=0.049).
